We are seeking a skilled RN Patient Care Manager to join our team at SunCrest Home Health - Durham, a part of LHC Group. As a Patient Care Manager, you will play a vital role in providing quality support to our patient-facing teams and ensuring the delivery of exceptional patient care.

Key Responsibilities

As a Home Health Patient Care Manager, you will be responsible for the supervision and coordination of clinical services. Your duties will include:
  • Providing clinical services within the scope of practice, as defined by state laws governing the practice of nursing, in accordance with the plan of care and in coordination with other members of the healthcare team.
  • Receiving referrals, ensuring appropriate clinician assignments, evaluating patient orders, and plotting start of care visits.
  • Coordinating determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
  • Ensuring patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
  • Reviewing assessments and plans of care daily, per assigned workflow, and consulting clinicians with recommendations, as appropriate.

Qualifications

To be successful in this role, you will need to possess the following qualifications:
  • Current RN licensure in the state of practice.
  • Current CPR certification required.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
