We are seeking a skilled Registered Nurse to join our Hospice team as an Admission Registered Nurse. In this role, you will be responsible for determining patient eligibility for hospice care and working closely with our case managers and social workers to ensure seamless transitions.
Key Responsibilities- Conduct initial assessments to determine patient eligibility for hospice care
- Work with case managers and social workers to develop individualized care plans
- Collaborate with insurance representatives to ensure accurate benefits coverage
- Participate in performance improvement initiatives to enhance patient care
- Current state registered nurse license
- Graduate of an accredited school of nursing
- Preferred experience in acute care, long-term care, home health care, community health, or public health
