Hospice Nurse Care Manager

About This Opportunity:
Croix Hospice is dedicated to providing end-of-life care to patients and their loved ones. As a Registered Nurse - Hospice Case Manager, you will be part of a team that prioritizes compassion, empathy, and community engagement.
Your Role:
As a RN Hospice Case Manager Specialist, you will be responsible for planning, organizing, and directing hospice care services in the community. You will work closely with patients, families, and healthcare teams to assess their needs and develop personalized care plans.
Key Responsibilities:
- Conduct comprehensive assessments of patients and families to determine their hospice care needs.
- Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their loved ones.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Provide education and support to patients and families on end-of-life care, symptom management, and bereavement.
- Document all patient interactions, care plans, and outcomes in accordance with regulatory requirements.

Requirements:
- RN licensure for the state(s) you work in.
