Hospice Liaison Specialist

About the Role:
The Community Liaison will serve as a key ambassador for Hospice of the Midwest, fostering relationships with healthcare professionals, community organizations, and individuals to promote our home health services.
Key Responsibilities:
- Establish and maintain professional relationships with referral sources, including hospitals, physician practices, nursing homes, and assisted living facilities.
- Ensure compliance with state, federal, and local regulatory requirements.
- Educate the community about home care services and Abode Home Health.
- Conduct outside sales calls to existing and potential referral sources within an established territory.
- Promote Abode Home Health's programs and services to medical professionals and civic organizations.
Requirements:
- Bachelor's degree in Business, Healthcare, or related discipline preferred.
- Current and valid state driver's license.
- Proof of auto insurance.
