We are seeking a skilled and compassionate Case Manager to join our interdisciplinary team. In this role, you will work closely with patients, families, and healthcare professionals to develop and implement individualized care plans that promote optimal outcomes.

Key Responsibilities
  • Conduct thorough assessments of patients' needs and develop comprehensive care plans that address their physical, emotional, and social needs.
  • Collaborate with healthcare professionals to ensure seamless care transitions and effective communication.
  • Provide education and support to patients and families to promote self-management and independence.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed.
  • Participate in interdisciplinary team conferences to discuss patient care and develop strategies for improvement.
Requirements
  • Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist.
  • Certification in Case Management or Rehabilitation Nursing preferred.
  • Minimum of 2 years social work or case management experience in an inpatient setting highly preferred.
  • Effective oral and written communication skills in English, additional languages preferred.
  • Basic computer skills in excel, word, outlook, power point, etc. required.
