We are seeking a skilled RN Case Manager or MSW Case Manager to join our team at Roper St. Francis Healthcare. As a key member of our Case Management department, you will play a vital role in coordinating patient care to achieve positive clinical and financial outcomes.
Key Responsibilities- Collaborate with interdisciplinary teams to execute responsibilities, including post-acute referrals, discharge planning, crisis intervention, counseling, and psychosocial assessments.
- Advocate for patients and families to ensure their needs are met, and provide accurate, timely documentation of all social work interventions in the medical record.
- Develop and update discharge plans to ensure they are consistent and up-to-date based on patient needs, preferences, and available resources.
- Provide supportive counseling to patients and families about placement options, including available resources, adoption, and guardianship.
- Ensure mandatory reporting and provision of discharging planning for patients in cases of suspected abuse or neglect.
- Master's Degree in Social Work (MSW) or Bachelor's Degree in Nursing (RN) from an accredited institution.
- At least 1 year of clinical acute care experience as an RN or Case Manager, preferably within the last 5 years.
- Currently licensed as a Registered Nurse in the state of South Carolina or holds a current compact/multi-state license as a Registered Nurse in a recognized NCSBN Compact State.
