About the Role

We are seeking a highly skilled and compassionate Patient Care Manager to join our team at Heartland Hospice.

As a Patient Care Manager, you will be responsible for the day-to-day operations of our hospice site, ensuring the provision of high-quality care to our patients and their families.

You will work closely with our interdisciplinary team to develop and implement individualized care plans, manage clinical and administrative functions, and provide leadership and guidance to our staff.

This is a challenging and rewarding role that requires strong clinical and leadership skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities
  • Manage the day-to-day operations of the hospice site, including clinical and administrative functions
  • Develop and implement individualized care plans for patients and their families
  • Provide leadership and guidance to staff, including clinical and non-clinical personnel
  • Collaborate with the interdisciplinary team to ensure high-quality patient care
  • Manage and oversee clinical and administrative recordkeeping functions
  • Respond to phone referrals and inquiries in a timely and professional manner
  • Participate in on-call nursing schedule and provide direct skilled bedside nursing care as needed
Requirements

To be successful in this role, you will need:

  • A minimum of three years' experience in direct patient care in a clinical setting
  • A minimum of one year of management/supervisory experience
  • Current license to practice nursing in the state of employment
  • CPR certification
  • Current automobile insurance and valid driver's license
