Job Summary

We are seeking a Patient Care Manager to join our team. As a key member of our hospice team, you will be responsible for the day-to-day operations of your assigned hospice site, ensuring the provision of quality care to patients. You will work closely with our Executive Director, Administrator, or Senior Patient Care Manager to manage the site and oversee clinical and administrative functions.

Responsibilities
  • Manage the day-to-day operations of your assigned hospice site
  • Administer the clinical aspects of the hospice program
  • Ensure the provision of quality care to patients
  • Manage and oversee clinical and administrative functions
  • Respond to phone referrals and inquiries in a timely and professional manner
  • Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes
Requirements
  • Registered Nurse with a minimum of Associate's Degree in Nursing required
  • Three years' experience in direct patient care in a clinical setting required
  • Minimum one year of management/supervisory experience required
  • Two or more years' experience in hospice or home care required
  • Current license to practice nursing in the state of requested employment
