Key Responsibilities- Coordinate the delivery of patient care both in and outside of the center.
- Plan, implement, and evaluate formal and informal educational activities to meet the identified health needs of patients.
- Demonstrate knowledge of available equipment and select appropriately for individual patient needs; evaluate and make recommendations for changes in equipment and related products utilized in wound and skin care management.
- Demonstrate knowledge of current technology and products used in the treatment of wounds.
- Coordinate patient care with the inpatient wound and skin care nurse.
- Meet performance expectations for customer service, teamwork, resource utilization, and staff and self-development as outlined in performance review.
- Maintain confidentiality of all hospital, staff, and patient information.
- Chart in electronic medical record.
- Participate in patient and family teaching.
- Maintain environmental and infection control standards.
- Perform other duties as assigned or directed to ensure smooth operation of the department/unit.
- Graduate of an approved school of nursing; BSN required within three years of assuming position.
- Minimum of three (3) years nursing experience, with one (1) year of dedicated wound care experience required. Hyperbaric experience preferred. Outpatient experience preferred. Wound care certification required, or within one year of hire.
- Must have a current State of Connecticut nursing license. BLS required.
