Job Description

At Peoples Family of Corporations, we are committed to providing comprehensive mental health services to children and adults in our community. Our team of professionals works diligently to provide care to patients and their families.

Key Responsibilities:

  • Provide community resource coordination and interactions to enable individuals at risk of hospitalization to receive support services.
  • Transport clients to appointments and conduct home and school visits.
  • Work with individuals who have been discharged from inpatient treatment to maintain their adjustment to living in the community.
  • Evaluate client status, including personal strengths, areas of concern, and needs.
  • Participate in formal assessments and develop person-centered plans.
  • Monitor client progress towards goal attainment and provide assistance as needed.
  • Train clients in living skills, social skills, and other pro-social activities to enable them to actively participate in the recovery process.

Requirements:

  • Bachelor's degree in Social Work, Psychology, or a related field.
  • Or Associate's degree and 2 years of experience in Community Mental Health, Case Management, Intensive In-Home services, or Social Work.
  • Or 4 years of experience in Community Mental Health, Case Management, Intensive In-Home services, or Social Work.
  • Valid Class E driver's license, dependable transportation, and personal vehicle insurance.
