Senior Patient Care Manager

We are seeking a highly skilled and compassionate Senior Patient Care Manager to join our team at Gentiva Hospice. As a key member of our leadership team, you will be responsible for overseeing the day-to-day operations of our hospice sites, ensuring the delivery of high-quality patient care, and managing a team of healthcare professionals.
Key Responsibilities- Supervise and manage the performance of assigned team members, ensuring they meet the highest standards of patient care and service.
- Administer the clinical aspects of the hospice program, including managing patient admissions, evaluations, and care plans.
- Oversee the management of clinical and administrative functions, including recordkeeping and compliance with regulatory guidelines.
- Respond to phone referrals and inquiries in a timely and professional manner, providing compassionate support to patients and their families.
- Participate in on-call nursing schedules and provide direct skilled bedside nursing care as needed.
- Degree in nursing with a current license to practice in the state of employment.
- Three years of experience in direct patient care, with at least one year of management experience.
- Two or more years of experience in hospice or home care.
- Excellent patient assessment skills, good oral and written communication, and thorough knowledge of managed care principles and regulatory guidelines.
