About the Role

We are seeking a skilled Physical Therapist to join our team at Collage Home and Community Rehabilitation Partners. As a Neurological Rehabilitation Therapist, you will play a vital role in providing exceptional care to our patients with neurological injuries or disorders.

Key Responsibilities
  • Evaluate and treat impairments, including lower extremity function, mobility, vestibular skills, adaptive equipment needs, and environmental safety needs.
  • Develop and implement individualized treatment plans to help patients achieve their functional goals.
  • Collaborate with interdisciplinary teams to create integrated treatment plans that address the unique needs of each patient.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
Requirements
  • Completion of a Bachelor's degree in Physical Therapy and state licensure.
  • Experience in the treatment of neurologically related disorders.
  • Excellent planning, organization, writing, and time management skills.
  • Ability to communicate effectively with diverse groups, including families, professionals, and co-workers.
