We are seeking a skilled Physical Therapist to join our team at Collage Home and Community Rehabilitation Partners. As a PRN Physical Therapist, you will have the opportunity to provide patient-centered care in a dynamic and growing organization.

Key Responsibilities
  • Evaluate and treat impairments, including lower extremity function, mobility, vestibular skills, adaptive equipment needs, and environmental safety needs.
  • Develop and implement individualized treatment plans to address the unique needs of each patient.
  • Collaborate with interdisciplinary teams to create integrated treatment plans that promote durable outcomes.
  • Communicate effectively with patients, families, professionals, and co-workers to ensure seamless care coordination.
Requirements
  • Completion of a Bachelor's degree in Physical Therapy and state licensure.
  • Experience in the treatment of neurologically related disorders.
  • Excellent planning, organization, writing, and time management skills.
