Palliative Care Nurse Practitioner Job Description

Job Summary: We are seeking a highly skilled Palliative Care Nurse Practitioner to join our team at Amedisys Inc. The successful candidate will provide high-quality, patient-centered care to individuals with serious illnesses, focusing on alleviating symptoms, pain, and stress.

Key Responsibilities:

  • Provide palliative care to patients in their homes or clinical settings, collaborating with physicians and medical directors as needed.
  • Conduct medically necessary assessments, documenting findings accurately and timely.
  • Schedule visits based on medical assessment and individualized care plans.
  • Order and review treatments, lab tests, and diagnostic procedures as indicated by the palliative plan of care.
  • Communicate with patients, families, and healthcare teams to ensure optimal care delivery.
  • Collaborate with business development, hospice, and home health units to promote palliative education and care coordination.

Requirements:

  • Master's degree in Nursing or higher.
  • Licensed Nurse Practitioner in the state where services are provided.
  • Certified as a Nurse Practitioner by the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
  • Authority to practice and prescribe as a nurse practitioner in the state where services are provided.
  • Medicare billing privileges as a nurse practitioner.

Preferred Qualifications:

  • Knowledge of disease-specific local coverage determinant (LCD) criteria for hospice eligibility and CMS Face to Face regulatory requirements.
  • Knowledge of hospice and home health philosophy and service components.
  • Knowledge of Medicare, Medicaid, and other payers' reimbursement and coding for all levels of service.
