About Us

Inland Empire Health Plan (IEHP) is a non-profit health plan dedicated to improving the health and well-being of our members. We are seeking a highly motivated and compassionate Care Manager to join our team.

Job Summary

This is a challenging and rewarding role that requires strong communication and organizational skills. The successful candidate will have experience in care management, nursing, or a related field and be able to effectively work with members, providers, and other stakeholders to deliver high-quality care management services.

Responsibilities

  • Collaborate with the Enhanced Care Management team to develop and implement effective care plans for members with complex medical needs.
  • Provide education and support to members and their families on disease management, healthy behaviors, and self-care techniques.
  • Participate in interdisciplinary care teams to coordinate member care and services.

Requirements

  • Degree in Nursing from an accredited institution required; Bachelor's degree preferred.
  • Active, unrestricted, and unencumbered Registered Nurse (RN) license issued by the California BRN required.
  • Three (3) or more years of care management experience in a healthcare delivery setting required.

Compensation

The salary range for this position is $43.87 - $58.13 per hour. IEHP offers a comprehensive benefits package, including medical insurance, dental and vision coverage, 457(b) option, generous paid time off, state-of-the-art fitness center on-site, pet care insurance, and flexible spending account.
