Job Title: Therapy Care Specialist

We are seeking an experienced THERAPY CARE SPECIALIST to join our team at Neogen Care in Rancho Cucamonga, CA.

Salary: $70,000 - $90,000 per year, depending on experience

Job Description:

The ideal candidate will have a strong background in occupational therapy, with a focus on home health rehabilitation. The successful candidate will conduct physical and psychological assessments, develop treatment plans, and educate patients and caregivers on adaptive equipment and techniques.

Key Responsibilities:

  • Conduct thorough assessments of patient needs and develop personalized treatment plans
  • Collaborate with multidisciplinary teams to ensure comprehensive patient care
  • Provide education and training to patients and caregivers on adaptive equipment and techniques
  • Maintain accurate records and documentation of patient progress
  • Participate in continuous quality improvement initiatives to enhance patient outcomes

Required Qualifications:

  • Bachelor's degree in Occupational Therapy from an accredited institution
  • Certification in occupational therapy from the National Board for Certification in Occupational Therapy
  • Valid California state license as an occupational therapist
  • 1+ years of experience in home health rehabilitation or a related field
