Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a Home Health RN Case Manager. As a key member of our interdisciplinary team, you will be responsible for creating and implementing individualized plans of care to promote wellness and prevent re-hospitalization for patients in their homes.

Key Responsibilities
  • Conduct comprehensive assessments of patients and their families to identify needs and develop effective care plans
  • Collaborate with physicians and other healthcare professionals to deliver high-quality patient care
  • Provide education and support to patients and their families on wound care, medication management, and other health-related topics
  • Manage patient care episodes and coordinate with interdisciplinary teams to ensure seamless care transitions
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Current and unrestricted California RN license
  • Current BLS/CPR certification
  • Valid California driver's license
  • High level of clinical knowledge in home health care
  • Experience working with high-acuity patients and interdisciplinary teams
