About the Role:

We are seeking a skilled Physical Therapist to join our team at At Home Therapies. As a Physical Therapist, you will provide comprehensive physical therapy evaluations and treatment plans to patients in our assisted living facility.

Responsibilities:

  • Conduct thorough physical therapy evaluations based on physician orders.
  • Develop and implement comprehensive treatment plans, including long-term and short-term goals, frequency, duration, and treatment modalities.
  • Ensure physician orders are obtained for evaluations, treatments, re-certifications, and discharges.
  • Provide high-quality treatment to patients, utilizing various modalities and modifying treatment as needed while adhering to precautions.
  • Complete all required documentation.

Requirements:

  • Graduate of an accredited university with a B.S., Doctorate, MA, or MS in Physical Therapy recognized by the APTA.
  • Current license and/or registration as a Physical Therapist in-state as applicable.
