Case Manager

At Behavioral Health Group, we are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in assisting patients in achieving their social, educational, and occupational goals.
Key Responsibilities:- Conduct regular case management sessions with patients to assess their needs and develop personalized treatment plans.
- Orient patients to treatment program operations and procedures, ensuring a smooth transition into our care.
- Collect and assess social, educational, and vocational information to inform treatment plans.
- Develop and revise patient treatment plans, maintaining comprehensive records and adhering to regulatory requirements.
- Make appropriate referrals and perform follow-ups to ensure continuity of care.
- Assist in implementing new programs and advising on caseload management to optimize patient outcomes.
- Provide crisis intervention and inter-agency documentation to support patients in crisis.
- Coordinate care with community providers to ensure seamless transitions and optimal patient care.
- Achieve annual goals and develop knowledge of addiction and related regulations to stay up-to-date in the field.
- Participate in in-service trainings and attend relevant conferences to enhance professional development.
- Adhere to all federal, state, and local regulations and accrediting agency requirements to ensure compliance.
- Participate in community and public relations activities to promote the organization's mission and values.
- Commit to diversity and inclusivity in the workplace, fostering a culture of respect and empathy.
- Bachelor's degree in a related Human Services field.
- Pursuing licensure as a Certified Alcohol and Drug Counselor (CADC) is preferred.
- Compliance with federal and state certification and licensure regulations.
- Knowledge of addiction, treatment methods, and current trends in opioid maintenance treatment is preferred.
