Job Summary:

As a Discharge Planner at Appalachian Regional Healthcare, you will play a vital role in ensuring a smooth transition of patients from acute care to community-based care. This position requires a strong understanding of case management principles and the ability to work collaboratively with patients, families, and healthcare teams to develop and implement effective discharge plans.

Key Responsibilities:

• Perform comprehensive assessments to identify patient needs and develop individualized discharge plans
• Collaborate with patients, families, and healthcare teams to ensure continuity of care
• Arrange for post-acute care services, including home health, rehabilitation, and skilled nursing facilities
• Provide education and support to patients and families to promote self-management and independence
• Participate in care conferences and multidisciplinary teams to coordinate patient care
• Maintain accurate and timely documentation of patient information and discharge plans

Requirements:

• Bachelor's degree in Social Work, Psychology, Sociology, or a related field
• Experience in case management or a related field
• Strong communication and interpersonal skills
• Ability to work in a fast-paced environment and prioritize multiple tasks
• Proficiency in Windows, Word, and other basic computer skills
