Job Summary

As a Case Manager at Behavioral Health Group, you will play a vital role in helping individuals overcome substance use disorders. You will work closely with patients to achieve their social, educational, and occupational goals, providing referrals, coordinating care, and adhering to all regulatory requirements.

Key Responsibilities
  • Conduct regular case management sessions to support patients in achieving their goals.
  • Orient patients to treatment program operations and procedures, ensuring a smooth transition into care.
  • Collect and assess social, educational, and vocational information to inform patient treatment plans.
  • Make appropriate referrals and perform follow-ups to ensure patients receive the necessary support.
  • Assist in implementing new programs and advise on caseload management to optimize patient outcomes.
  • Provide crisis intervention and inter-agency care with community providers to ensure patients receive comprehensive support.
Requirements
  • Bachelor's degree in a related Human Services field.
  • Pursuing licensure as a Certified Alcohol and Drug Counselor (CADC) is preferred.
  • Compliance with federal and state certification and licensure requirements.
