Position Overview:

As a vital member of the healthcare team, the Advanced Practice Provider (APP) collaborates closely with hospitalists to deliver comprehensive patient care, encompassing diagnosis, treatment, and consultations. This role is integral to both independent practice and collaborative treatment teams, providing education and case management while serving as a consultant to multidisciplinary healthcare professionals.

Key Responsibilities:

  • Deliver clinical care and conduct thorough patient evaluations for individuals in the hospital.
  • Establish diagnoses and formulate treatment plans for various health conditions.
  • Order and interpret medical diagnostics in conjunction with attending physicians.
  • Prescribe medications, therapies, and medical equipment as necessary.
  • Provide patient education to effectively manage chronic illnesses and promote wellness.
  • Refer patients to specialty care when appropriate.
  • Consult with physicians for cases that exceed APP scope of practice or when treatment plans are ambiguous.
  • Engage actively with the healthcare team and serve as a resource for team members.
  • Collaborate with primary care teams to enhance program development.
  • Act as a liaison with external agencies as required.
  • Foster effective communication within the team and among interdisciplinary healthcare staff.
  • Perform additional duties as assigned.
Practice Environment
  • Supportive and collaborative atmosphere with the Hospitalist team.
  • Utilization of Excellian, an advanced EMR system recognized for its comprehensiveness.
  • Consideration for both Physician Assistants and Nurse Practitioners.
  • Full-time schedule with a 7 on 7 off rotation, covering day and evening shifts.


Qualifications:

  • Master's degree from an accredited Physician Assistant or Nurse Practitioner program.
  • Certification as a Physician Assistant from NCCPA or as a Nurse Practitioner through ANCC.
  • Possession of an active, unrestricted medical license in Minnesota (or eligibility to obtain one).
  • Ability to secure and maintain MN DEA certification and National Provider Identifier (NPI).
  • Meet the criteria for credentialing and hospital privileges.
  • ACLS/BLS Tier 3 certification required within 180 days.
  • Preferred: One year of experience as a Physician Assistant or Nurse Practitioner.
