Key Responsibilities:
  • Evaluate patient responses to medication regimens and develop tailored drug therapy plans based on established medical diagnoses.
  • Order and interpret laboratory tests to assess the effectiveness of pharmacotherapy.
  • Initiate, adjust, or terminate medications for the management of chronic health conditions, collaborating closely with medical professionals.
  • Oversee and adjust pharmacotherapy that requires regular modifications due to varying pharmacokinetic properties.
  • Dispense prescription and over-the-counter medications in accordance with established protocols.
  • Assess nutritional needs and prescribe appropriate enteral or parenteral nutrition in inpatient settings.
  • Review medical and medication histories to identify and resolve drug-related issues.
  • Collaborate with healthcare teams, including physicians and nurses, to optimize patient care.
  • Conduct comprehensive pharmacist procedures and ensure accuracy in medication dispensing.
  • Review patient profiles during medication dispensing to monitor therapy closely.
  • Verify that prescription instructions, dosages, and potential drug interactions are suitable for each patient.
  • Ensure the precision of all pharmaceutical products prepared by technical staff.
  • Provide training to clinical staff on new medications, their appropriate usage, and relevant therapeutic comparisons.
  • Document significant medication interactions and maintain communication with prescribers.
  • Conduct Adverse Drug Reaction (ADR) reporting in line with accreditation standards.
  • Assist in medical staff drug utilization reviews as necessary.
Essential Qualifications:
  • Citizenship: Proof of U.S. Citizenship is required.
  • Education/Certifications:
    • Doctor of Pharmacy (Pharm.D.) from an accredited institution.
    • Active licensure from a U.S. state or territory.
    • Completion of a clinical pharmacy internship or residency accredited by relevant professional organizations.
    • Current, unrestricted license to practice as a clinical pharmacist.
    • Additional privileges may require certification in recognized practice areas or completion of relevant postgraduate education.
    • Must meet Basic Life Support (BLS) requirements.
  • Experience:
    • Recent completion of an accredited pharmacy residency program or relevant clinical experience in a patient care setting.
    • Minimum of two years of direct patient care experience as a pharmacist within the last three years.

$157,000 USD
