Responsibilities- Assessment and Evaluation: Assess and evaluate the health care needs of patients and families, considering physiological, psychological, social, and environmental factors.
- Care Planning: Participate in mandatory comprehensive care planning meetings and collaborate with the interdisciplinary team to develop a comprehensive care plan based on member-specific needs.
- Nursing Care: Implement the nursing care plan and revise it as necessary, regularly assessing, observing, and evaluating the patient's condition, needs, and response to care.
- Medication Administration: Administer medications and treatments as prescribed by the physician and independently perform skilled nursing procedures and techniques.
- Patient Education: Teach patients and families/caregivers self-techniques and provide instruction regarding medication, diet, safety, and treatment modalities in accordance with the plan of care.
- Documentation: Complete, maintain, and submit all required documentation in a timely, accurate, and relevant manner.
- Education: Degree from an NLN accredited school of nursing required, BSN preferred.
- License: Current New York State license to practice as a Registered Nurse.
- Experience: Minimum of one (1) year general clinical nursing experience in a certified home health agency (CHHA), acute care, medical surgical, and/or critical care experience required.
- Population Focus: Minimum of one (1) year of experience working with a frail or elderly population preferred.
