About the Role

We are seeking a seasoned RN Care Coordinator to join our team at LifeBridge Health. As a key member of our healthcare team, you will play a critical role in managing individualized patient-centered care through the use of the nursing process and primary nursing principles.

As an RN Care Coordinator, you will be responsible for coordinating care for patients with complex needs, including those with uncontrolled chronic conditions and rising risk indicators. You will work closely with providers, nurses, and community resources to expedite medically appropriate cost-effective care.

The ideal candidate will possess strong analytical, data management, and computer skills, as well as high-level verbal and communication skills. A bachelor's degree in nursing is required, and a master's degree is preferred. Three to five years of demonstrated proficiency in acute care nursing, knowledge, and skills are also necessary.

Key Responsibilities

  • Performs standardized comprehensive needs assessments to determine available benefits and resources.
  • Develops and implements plans of care for assigned patients that include performance goals, monitoring, follow-up, and outreach activities.
  • Interfaces with providers, nurses, and community resources to facilitate timely access to appointments and services.
  • Evaluates the effectiveness of care plans with interdisciplinary teams.

Requirements

  • Graduate of an accredited school of nursing; Bachelor's degree required, Master's degree preferred.
  • Current/valid registered nurse license in the State of Maryland or Compact State as applicable.
  • Case/care management certified or knowledge of national care management standards and community resources a plus.
  • Three to five years demonstrated proficiency in acute care nursing, knowledge, and skills.

Additional Information
