About the Role
We are seeking a dedicated Healthcare Navigator and Care Coordinator to join our team at LifeBridge Health.
Job Summary:
The Medical Respite Care Coordinator/Navigator will be an essential member of our integrated healthcare team, supporting medical respite program goals of improving pathways to healthcare for at-risk residents of Maryland.
- As a system navigator and care coordinator, you will work closely with medical respite patients to ensure they receive comprehensive care, including primary and specialty healthcare, dental/oral health, behavioral health, diagnostic services, and social services.
- You will participate in triage and intake processes, providing intensive case management and advocacy for at-risk patients, including resource navigation, application assistance, transportation, social supports, and housing stability.
- Your strong relationships with other providers and services within the community will enable you to provide person-centered care, ensuring seamless coordination of services.
- With a working knowledge of State of Maryland and Federal assistance programs, including public insurances, financial assistance gap programs, Social Security Administration, food stamps, energy assistance, etc., you will aid patients in navigating these complex systems.
- You will also participate in community workgroups and continuing education opportunities for patient advocacy, healthcare advancements, and entitlement programs.
- In addition, you will conduct needs assessments to identify barriers to obtaining healthcare services, available resources, and ultimately diversion of over utilization of emergency department services.
- As a valuable member of our team, you will provide clinical and procedural support training to other members of the healthcare team related to care coordination and navigation processes.
- Further, you will participate in the system development and reporting of quality assurance/performance improvement measures and activities.
- You will coordinate referrals, specialty visits, and diagnostic services with proper documentation and patient education.
- You will track care coordination activities via patient registries and specialized databases.
- Lastly, you will provide monthly reports of statistics, including care coordination, referrals, individuals served, and relevant outcomes.
About You
To succeed in this role, you will possess a Licensed nurse in State of Maryland, preferably with Human Services field experience. A Bachelors Degree from an accredited college or university is preferred, but not required. Current BLS Certification from the American Heart Association is a must.
