Job Summary

We are seeking a highly skilled and compassionate Telemedicine Care Coordinator to join our team at Concentra. As a Telemedicine Care Coordinator, you will play a vital role in providing exceptional care to our patients and making a meaningful difference in their lives.

Key Responsibilities
  • Welcome patients and obtain medical histories from patients, verifying patient information as needed.
  • Coordinate patient care through video intake calls, ensuring seamless communication and coordination with Telemedicine Physicians or Concentra centers.
  • Collect and document patient information, including demographics, chief complaint, review of medications, and allergies.
  • Screen and triage patients based on intake questions, routing them to the appropriate care.
  • Perform administrative duties as needed, including scheduling appointments, contacting patients and customers, and answering calls.
  • Document patient information in electronic medical records, maintaining accurate and up-to-date records.
Requirements
  • High School Diploma or GED.
  • At least one year of experience with patient interaction via phone and video, preferably in a healthcare setting.
  • At least one year of working in a healthcare office environment, with a strong understanding of medical office administration and procedures.
  • Bi-lingual, with conversational fluency in both English and Spanish, a plus.
  • Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Ability to make decisions and solve problems using logic and critical thinking.
  • Excellent time management and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities.
