Job Description

PATH is seeking a skilled Case Manager III to join our Interim Housing Services team at the Yale Navigation Center. As a key member of our team, you will play a vital role in supporting clients through all phases of homelessness, from the streets to permanent housing.

Key Responsibilities:

  • Conduct intake and individualized needs assessments for clients, developing personalized service plans to address housing goals, income, and other personal objectives.
  • Provide referrals, linkages, information, and support to resources that help clients achieve their service plan goals.
  • Complete program-specific assessments, such as the VI-SPDAT, to evaluate client strengths and challenges.
  • Maintain accurate and up-to-date client files, adhering to PATH policies and documentation standards.
  • Ensure confidentiality of client files, adhering to HIPAA guidelines and all applicable regulations.

Requirements:

  • Two years of experience in healthcare, case management, homeless services, or a related field.
  • Reliable transportation, a valid driver's license, and proof of insurance and vehicle ownership.
  • Ability to qualify for PATH's insurance coverage.
