Job Summary

This position is responsible for providing psychosocial rehabilitation services to clients with serious mental illness (SMI) in an outpatient setting. The ideal candidate will have experience working with SMI, chronic substance dependence, or co-occurring diagnoses, and be able to provide classes/activities with a focus on rehabilitation and improving the quality of life for adults to live as independently as possible.

Key Responsibilities
  • Provide clients with daily classes/activities with psychosocial rehabilitation interventions designed to address functional limitations, deficits, and behavioral excesses.
  • Efficiently and accurately complete all required documentation, including progress notes, initial assessment, initial treatment plan, and treatment plan reviews.
  • Work as part of a treatment team to provide comprehensive services to clients.
Requirements
  • High School Diploma or equivalent and one (1) year experience as it pertains to the essential duties and responsibilities of the classification.
  • Experience working as part of a treatment team and coordinating programs.
  • Ability to work with clients who may exhibit unpredictable and potentially violent behavior.
Working Conditions

This position will perform in an outpatient hospital and office setting with exposure to Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage. The position is located at Community Based Services and the shift is 7:30 am to 3:30 pm.

Supplemental Information

This position is covered by a collective bargaining agreement and all terms/conditions of that agreement apply and must be adhered to.
