Physician - Nashville

We are seeking a highly skilled and compassionate Primary Care Physician to join our team at CenterWell Senior Primary Care. As a key member of our care team, you will play a vital role in delivering high-quality, patient-centered care to our seniors.
Responsibilities- Evaluate and treat patients in accordance with established standards of care.
- Follow evidence-based medical guidelines and quality metrics to ensure optimal patient outcomes.
- Collaborate with our interdisciplinary care team to coordinate patient services, including specialty referrals, hospital and SNF coordination, and durable medical equipment.
- Participate in daily huddles and contribute to the development of care plans that address the physical, emotional, and social needs of our patients.
- Work closely with our Regional Medical Director and Center Administrator to promote a culture of cooperation and excellence in practice.
- Document patient care in a timely and accurate manner, working with our quality-based coder to optimize coding specificity.
- Stay up-to-date with policy and protocol changes, and participate in ongoing education and training to enhance your skills and knowledge.
- Current and unrestricted medical license in the state of practice, with the ability to obtain licenses in other states as needed.
- Graduate of an accredited MD or DO program.
- Excellent verbal and written communication skills, with the ability to build strong relationships with patients, families, and colleagues.
- Demonstrated expertise in integrated team-based care, with a commitment to delivering high-quality, patient-centered care.
- Ability to learn and adapt to our value-based care model, with a focus on quality, safety, and patient satisfaction.
- Basic computer skills, including email and electronic medical records.
- Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Active and unrestricted DEA license.
- Medicare Provider Number and Medicaid Provider Number.
- Minimum of two to five years of directly applicable experience in a value-based care setting.
- Experience managing Medicare Advantage panels, with a strong understanding of best practices in coordinated care environments.
