As a key member of the Yale New Haven Health team, you will play a vital role in coordinating the scheduling and data management for patients referred for advanced cardiac therapies. This includes maintaining accurate and up-to-date information on candidates waiting for transplantation, as well as ensuring effective communication with the transplant and VAD coordinator team.

The ideal candidate will possess excellent organizational skills, with the ability to handle multiple ongoing tasks and prioritize responsibilities. Database management skills are preferred, and the ability to interact with all levels of staff, patients, and families is essential.

Responsibilities:

  • Coordinate evaluation for patients referred for advanced cardiac therapies
  • Maintain the transplant waiting list to ensure accuracy of patient information
  • Interface with other hospital departments and personnel to ensure pertinent documentation has been communicated
  • Work closely with the staff and Histocompatibility Lab to document blood types and information for listing patients on the national registry

Qualifications:

  • Associates Degree in Allied Healthcare or related field preferred
  • Two years of related clinical experience or equivalent combination of education and experience
  • Excellent organizational skills, with the ability to handle multiple ongoing tasks and prioritize responsibilities

Additional Information:

  • Previous experience as a transplant associate coordinator is highly valued
  • Flexible hours and a great team player are essential
  • Excellent time management and knowledge of Epic Phoenix, SiteCore, and Scheduling are preferred
