As a hospice LPN, you will have the opportunity to make a meaningful impact on the lives of patients and their families providing much needed care where they want to be - in their homes.
You will work in community-based care centers with a supportive and inclusive work environment, verifying plans of care with the RN prior to each visit, teaching patients and caregivers self-care techniques, reviewing all medications and performing medication reconciliations at each visit, and preparing equipment and materials for treatments using the aseptic technique.
