Hospice Nurse Specialist

Gentiva Hospice is seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN. As a Hospice RN, you will provide high-quality patient care and support to individuals and their families in need of hospice services.
Key Responsibilities- Provide direct patient care services to hospice patients and supportive care to the patient's family
- Act as primary liaison between physician, patient/family and hospice team
- Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient
- Manage and collaborate with an interdisciplinary group consisting of Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA) and support staff
- RN license in the state of requested employment (or eligibility to obtain)
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
- Current driver's license and automobile insurance
- Current CPR certification
