Job Title: Home Health Registered Nurse RN Full Time

Job Summary:

Aveanna Healthcare is seeking a skilled and compassionate Home Health Registered Nurse to join our team. As a Home Health Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, working closely with our clinical team to ensure the best possible outcomes.

Key Responsibilities:

  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
  • Provide skilled nursing care, including wound care, infusions, and catheter management, in patients' homes.
  • Conduct thorough assessments, identify patient needs, and develop effective care plans.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Participate in ongoing education and training to stay current with best practices and industry developments.

Requirements:

  • Active RN license in the state of application.
  • Valid CPR certification.
  • 1-year RN experience in a healthcare setting, preferably in home health care.

Preferred Qualifications:

  • Medicare skilled nursing experience.
  • Basic understanding of Oasis.
