Key Responsibilities:

  • Develop and implement comprehensive care plans in accordance with physician orders and patient needs.
  • Provide education and training to patients and their families on disease management, medication adherence, and self-care techniques.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care delivery.

Requirements:

  • Active RN license in the state of application.
  • Valid CPR certification.
  • 1 year of experience in a healthcare setting, preferably in home health or skilled nursing.
  • Basic understanding of Oasis and other relevant healthcare software.
