As a key member of our clinical team, you will provide high-quality nursing care to patients in their homes, working collaboratively with physicians and other healthcare professionals to develop and implement individualized care plans.
Key Responsibilities:- Develop and implement comprehensive care plans for patients, incorporating medical interventions and measurable goals and outcomes.
- Educate patients and their family members on specific care needs, promoting independence and self-management.
- Train primary caregivers to ensure optimal functioning levels for each patient, providing guidance and support as needed.
- Coordinate continuum of care through timely documentation and communication with physicians and other caregivers.
- Provide skilled nursing care in patients' homes, utilizing a range of skills including phone triage, patient education, observation/assessment, wound care, infusions, catheter care, and PICC line dressing changes.
