Job Title: Discharge Planner Supervisor

Central State Hospital is seeking a highly skilled Discharge Planner Supervisor to join our team. As a key member of our discharge planning team, you will be responsible for ensuring the smooth transition of patients from our facility to their communities.

Job Summary:

The Discharge Planner Supervisor will oversee the discharge planning process, ensuring that patients receive the necessary support and resources to successfully transition back to their communities. This includes collaborating with multidisciplinary teams, community providers, and patients to identify and address barriers to discharge.

Key Responsibilities:
  • Supervise and coordinate the discharge planning process, ensuring timely and effective transitions of patients.
  • Collaborate with multidisciplinary teams, including social workers, nurses, and physicians, to identify and address barriers to discharge.
  • Develop and maintain relationships with community providers, including hospitals, clinics, and residential facilities, to ensure seamless transitions.
  • Provide education and support to patients and their families on discharge planning and community resources.
  • Conduct regular reviews of discharge plans to ensure they are up-to-date and effective.
  • Collaborate with the Social Work Director to develop and implement discharge planning policies and procedures.
Requirements:
  • Master of Social Work (MSW) degree required.
  • Supervisory experience in case management and discharge planning preferred.
  • Knowledge of forensic mental health and experience working with the DBHDS state psychiatric hospitals or CSBs/BHAs is preferred.
