Job Summary

We are looking for an experienced Psychiatric Care Coordinator to join our team. As a key member of our interdisciplinary team, you will be responsible for coordinating care for patients in a psychiatric setting. Your expertise will enable us to deliver exceptional patient outcomes and enhance our reputation as a leader in healthcare staffing.

Key Responsibilities
  • Coordinate patient care across multiple settings, including inpatient and outpatient facilities
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless transitions and optimal care
  • Assess patient needs and develop individualized care plans that address complex psychosocial and medical requirements
  • Participate in quality improvement initiatives to enhance patient satisfaction and outcomes
Requirements
  • Current Registered Nurse licensure in-state practicing
  • Minimum 2 years of experience in psychiatric nursing or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a collaborative team
