Key Responsibilities- Provide expert clinical guidance and support to our healthcare teams, ensuring seamless care delivery and optimal patient outcomes.
- Collaborate with national leaders in clinic operations, legal, quality, referrals, care management, and home-based care programs to develop and enhance current workflows, leveraging evidence-based practice to improve efficiency in care delivery, documentation, reimbursement, and patient outcomes.
- Analyze Key Performance Indicator metrics utilizing EMR reports, working closely with population health and quality departments to improve various internal workflows and enhance patient outcomes.
- Develop and implement proposals for approval by the National Medical Informatics Committee, focusing on creating and enhancing current workflows that align with Value-Based Care delivery models.
- Provide training and support to physicians and advanced practice providers nationwide on documentation guidelines, EMR navigation, and current workflows that align with Value-Based Care delivery models.
- Active, unrestricted NP/PA license in the appropriate state.
- Current certification as an NP or PA.
- Current DEA license or ability to obtain a DEA license.
- Passion for contributing to an organization focused on continuously improving consumer experiences.
- Familiarity with Value-Based Care models.
- Outpatient senior care experience.
- Familiarity with HEDIS measures.
- Guaranteed base salary + biannual bonus.
