Clinical Health Specialist

Matrix Medical Network is seeking a skilled Clinical Health Specialist to join our team. As a Clinical Health Specialist, you will conduct comprehensive health assessments in the comfort of patients' homes, reviewing their medical history, medication adherence, social environment, and other risks. This role requires strong communication and interpersonal skills, as you will work collaboratively with patients, family members, and healthcare professionals to provide high-quality care.
Responsibilities:
Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
Provide health education and counseling to patients and their families
Collaborate with primary care physicians and other healthcare professionals to ensure seamless care
Work in a variety of settings, including home visits, skilled facilities, and telehealth
Requirements:
Master's degree in a relevant field or equivalent experience
Current RN and NP or PA licensure in the state of practice
Board certification in a Matrix-approved specialty
Strong computer skills and familiarity with EMR software
Excellent verbal and written communication skills
