Job Summary

We are seeking a highly skilled Medical Care Manager to join our team at Charter Oak Communities. As a key member of our healthcare staff, you will play a critical role in ensuring the highest quality of care for our residents.

Responsibilities
  • Patient Care Management:
  • Coordinate and oversee all direct medical care for residents under the supervision of the Nurse Manager (RN).
  • Conduct monthly assessments of blood pressure and weight, and communicate with residents about their health issues.
  • Transcribe doctor orders and implement necessary actions.
  • Provide LPN functions, including wound care, nebulizer treatments, diabetic testing, and flu vaccines.
  • Coordinate medical appointments, tests, and lab work as required.
  • Schedule transportation and assist with discharge planning.
  • Medication Administration:
  • Order resident medications and maintain accurate records.
  • Evaluate medication administration by direct care staff to ensure accuracy and safety.
  • Maintain and update medication administration records to comply with state regulations.
Requirements
  • State of Connecticut Licensed Practical Nurse (LPN) certification is required.
  • A minimum of four (4) years LPN experience, preferably two (2) in a managerial role.
  • Experience working with diverse populations and providing a range of services.
  • CPR certification is mandatory.

Estimated Salary: $65,000 - $80,000 per year.
