CNY Family Care, LLP is seeking a skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in a hybrid position. This role will involve providing professional assessments, prioritizing treatments, and initiating medical care to patients in our busy practice.
Key Responsibilities:- Respond to patient requests or medical advice questions from the patient portal and phone calls in a timely manner.
- Review and react to various patient documents, including surgeries, stress tests, CT scans, MRI results, Mammograms, Immunizations, etc.
- Update medications, problems, diagnosis, allergies, and history in the Electronic Medical Record from review of documents.
- Communicate with patients' Primary Care Physicians with any significant change in health status.
- Valid NYS Physician Assistant (PA) or Nurse Practitioner (NP) license.
- A minimum of two (2) years' experience working in a family practice or Urgent Care setting.
- Previous experience working with Medent (EMR) is preferred.
- Energetic and excellent working knowledge of chronic disease states, with critical thinking skills.
- Competitive compensation and benefits package.
