Job Description

The Hospice Care Coordinator at Southwest General will work closely with patients, families, and healthcare teams to provide exceptional care and support services. This role involves developing and implementing individualized care plans, collaborating with physicians, and ensuring comprehensive treatment.

Main Responsibilities
  • Patient Care Planning: Develop and implement individualized care plans that address patients' physical, emotional, and social needs.
  • Care Coordination: Collaborate with healthcare teams to ensure seamless care coordination and effective communication.
  • Social Support: Provide emotional support and connect patients with community resources to enhance their quality of life.
Required Qualifications

To be successful as a Hospice Care Coordinator at Southwest General, applicants must meet the following requirements:

  • Education: Bachelor's degree in Nursing from an accredited institution and current licensure by the Ohio State Board of Nursing.
  • Experience: Prior experience in hospice care or similar settings.
  • Certifications/Licenses: Current licensure by the Ohio State Board of Nursing and CPR certification; ACLS certification is preferred.
Estimated Annual Salary

$71,000 - $95,000, depending on experience and qualifications.
