The Clinical Care Coordinator RN provides skilled nursing services on an intermittent basis, effectively communicating patient progress with other disciplines and guiding hospice aide staff in personal care provision. Key responsibilities include:
- Admitting patients eligible for hospice services under physician's orders.
- Evaluating patient needs and identifying mutually agreed-upon goals, taking into account patient preferences and priorities.
- Reporting patient status and need for other disciplines to clinical leadership, attending physician, and hospice physician.
- Updating care plans regularly, revising and resolving patient problems and goals as changes occur.
- Completing informational visits and obtaining patient consents for hospice admission per office procedure.
- Ensuring Age-Friendly Care, focusing on the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility).
- A valid registered nurse license in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
- A valid driver's license and good driving record are required for field-based work.
- The ability to work independently and make sound nursing judgments is essential.
- The capacity to assess patient needs and formulate individualized patient care plans is critical.
Estimated annual salary range: $80,000 - $110,000
