Job Summary

We are seeking a dedicated and compassionate individual to join our team as a Community Support Specialist. In this role, you will provide direct support and guidance to individuals with disabilities, helping them to reach their maximum potential and promote independence and community integration.

Key Responsibilities
  • Provide guidance, training, and support to individuals in reaching their goals and promoting independence
  • Develop and implement individualized plans to meet the unique needs of each person
  • Collaborate with the care team to ensure comprehensive support and services
  • Document services and progress in a timely and accurate manner
  • Participate in team meetings and contribute to the development of individualized plans
Requirements
  • High school diploma or equivalent
  • Valid NYS driver's license with a safe driving record
  • Ability to lift 50 pounds and assist with personal care needs
  • Ability to work as part of an interdisciplinary team and maintain composure under pressure
Working Conditions

This role involves frequent walking on a college campus and may require evening and weekend work. The ability to drive safely and assist individuals with ambulation is also required.
