Job Summary

New Avenues to Independence, Inc. is seeking a highly motivated and dedicated Residential Coordinator to join our team. As a key member of our residential services team, you will play a vital role in ensuring the well-being and success of individuals with disabilities.

Key Responsibilities
  1. Assist in accessing medical and health-related services for individuals receiving services, providing on-call coverage and participating in rotational emergency coverage and crisis intervention support as needed.
  2. Assess referred individuals and participate in the development of a habilitation plan in conjunction with a treatment team and funding source, coordinating and evaluating plan implementation.
  3. Provide direction and supervision to assigned staff, including input into performance evaluation and discipline, and assist in the recruitment, screening, hiring, and evaluation of staff.
  4. Ensure appropriate staff coverage according to provided ratios within the IHP, coordinate staff schedules, and manage overtime numbers within budgeted limits.
  5. Ensure staff are properly trained on regular trainings at site, including annual trainings, health and safety alerts, individual-specific trainings, and any other training requirements.
  6. Implement and contribute to the development, review, and revision of programs and behavior plans, strategies, and guidelines.
  7. Schedule and/or provide opportunities for quality training, recreation, and leisure activities with consideration for choice, preference, and age-appropriateness.
  8. Provide monitoring, consultation, and comprehensive support services to staff regarding work with individuals supported and implementation of habilitation plans.
  9. Maintain current and complete client and staff reporting records in accordance with regulatory standards, assist with legal, licensing, regulatory, and certification activities as needed.
  10. Ensure documentation compliance by monitoring completion of Active Treatment documentation, Medication Administration Records, vehicle inspection reports, and logs.
  11. Ensure the overall operations of the house are completed and up to standards set forth within the policies of New Avenues to Independence, Inc.
  12. Maintain current and complete client records and reports in accordance with corporate and regulatory standards.
  13. Ensure proper reporting of any Unusual Incidents, report all Unusual Incidents to the Residential Services Manager, and follow county-specific reporting requirements for any Major Unusual Incidents.
  14. Maintain communication with governing/oversight bodies, day location managers, guardians, family members, and all interested parties.
  15. Assist the Residential Manager with maintaining benefit eligibility for all individuals with identified services of benefit maintenance in their Individual Service Plans.
  16. Maintain current inventories for all individuals at the site, log all required purchases, and remove items that are no longer needed.
  17. Ensure completion and accuracy of timesheets and mileage logs, and timely submission of timesheets in accordance with New Avenues to Independence procedures.
Requirements

Education and/or Experience: High School Diploma or General Equivalency Diploma required. Bachelor's Degree preferred. Must have one year of previous experience with individuals with developmental disabilities, residential experience preferred.

Certificates, Licenses, Registrations: Must maintain current certification in Crisis Prevention and Intervention (CPI), CPR/First Aid. Must hold a valid Ohio driver's license and possess a driving record that meets New Avenues to Independence, Inc. insurance carrier acceptability.
