Job Summary:

The Clinical Social Worker is a vital member of our healthcare team, responsible for providing comprehensive social work assessments and interventions to patients, families, and at-risk populations. This role requires a strong understanding of medical social work principles, as well as excellent communication and problem-solving skills.

Key Responsibilities:

  • Perform social work assessments and interventions for inpatients, outpatients, and emergency department patients
  • Develop and implement treatment plans that integrate medical, social, and resource issues
  • Collaborate with healthcare team members to develop and implement patient-centered plans of care
  • Provide crisis intervention, patient/family intervention, high-risk screening, brief counseling, and referrals for financial or other identified resource needs
  • Arrange and facilitate family/patient representative meetings with the healthcare team as needed
  • Assist in post-acute placement and complex discharges
  • Engage appropriate agencies or community resources for high-risk patients

Requirements:

  • At least one year of clinical and/or medical social work experience
  • Masters Degree from a School of Social Work accredited by the Council of Social Work Education
  • Knowledge of contemporary behavioral and systems theories relevant to healthcare
  • End-of-life dynamics and interventions
  • Grief and bereavement counseling
  • Substance abuse identification and interventions
  • Support for victims of abuse, neglect, or violence
  • Community or financial resources for underserved or vulnerable populations
  • Thorough comprehension of OBRA and Title 22
