About the Role:
The Clinical Social Worker is a vital member of our multidisciplinary healthcare team at CommonSpirit Health. As a skilled professional, you will be responsible for performing comprehensive assessments and developing treatment plans that integrate medical, social, and resource issues impacting individual patients, families, and at-risk populations.
Key Responsibilities:
- Perform social work assessments and interventions as needed for inpatients, outpatients, and emergency department patients.
- Develop and implement treatment plans that support patient-centered care for individual patients and the medical community.
- Collaborate with healthcare team members to identify and address high-risk patients, arrange and facilitate family/patient representative meetings, and engage appropriate agencies or community resources.
- Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families.
- Participate in hospital committees and develop/present in-services to hospital staff.
Requirements:
- At least two years of clinical and/or medical social work experience.
- Masters Degree from a School of Social Work accredited by the Council of Social Work Education.
- Knowledge of contemporary behavioral and systems theories relevant to healthcare, end-of-life dynamics, and interventions.
- Thorough comprehension of OBRA and Title 22.
Preferred Qualifications:
- Licensed Clinical Social Worker or Licensed Clinical Social Worker in process preferred in California.
