Job Summary

We're seeking a skilled RN Case Manager to join our team in Wayne and Macomb Counties. This is a work-from-home position with 25-50% travel requirements. You'll work closely with our interdisciplinary team to assess, plan, implement, and coordinate case management activities for our members.

Key Responsibilities
  • Conduct thorough assessments of members' medical needs and develop proactive plans of care to address identified issues.
  • Collaborate with supervisors and other stakeholders to overcome barriers in meeting goals and objectives.
  • Utilize clinical judgment to incorporate strategies designed to reduce risk factors and barriers.
  • Conduct assessments that consider information from various sources, such as claims, to address all conditions, including co-morbid and multiple diagnoses.
  • Apply case management processes in compliance with regulatory and company policies and procedures.
Requirements

To be successful in this role, you'll need:

  • A registered nurse license in good standing in Michigan.
  • Minimum 3+ years of clinical experience and 2+ years of care management experience.
  • Ability to travel 25-50% within Wayne and Macomb Counties.
  • Excellent analytical and problem-solving skills, effective communication, and interpersonal skills.
  • Ability to work independently and proficiency with standard corporate software applications.
