As a Case Manager RN, you will play a vital role in this mission, working closely with patients, healthcare providers, and community organizations to address complex health and social challenges.

Key Responsibilities:
  • Assess, plan, implement, and coordinate case management activities to evaluate the medical needs of patients and facilitate their overall wellness.
  • Develop proactive plans of care to address identified issues, enhancing short and long-term outcomes and opportunities for improved member wellness.
  • Utilize clinical tools and information/data review to conduct evaluations of patient needs and benefits.
  • Apply clinical judgment to incorporate strategies designed to reduce risk factors and barriers, addressing complex health and social indicators that impact care planning.
  • Conduct assessments considering information from various sources, such as claims, to address all conditions, including co-morbid and multiple diagnoses that impact functionality.
  • Collaborate with supervisors and other key stakeholders in the patient's healthcare to overcome barriers in meeting goals and objectives, presenting cases at interdisciplinary case conferences.
  • Utilize case management processes in compliance with regulatory and company policies and procedures.
  • Employ motivational interviewing skills to ensure maximum patient engagement and discern their health status and health needs based on key questions and conversation.
Requirements:
  • Registered Nurse with active MI state license in good standing.
  • Minimum 3+ years clinical practical experience required.
  • Minimum 2+ years Care Management, discharge planning, and/or home health care coordination experience required.
  • Ability to travel 25-50% within Wayne and Macomb Counties.
Preferred Qualifications:
  • Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually.
  • Bilingual – Arabic or Spanish.
  • Past experience with Medicaid or the Waiver population.
  • If residing in a compact licensure state, it is preferred that RN license is part of the compact.
  • Certified Case Manager is preferred.
  • Excellent analytical and problem-solving skills.
  • Effective communications, organizational, and interpersonal skills.
  • Ability to work independently.
  • Effective computer skills including navigating multiple systems and keyboarding.
  • Demonstrates proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as some special proprietary applications.
